Welcome to our Acupuncture & Chinese herbal medicine practice, Live Well Medicine.  Please take a few minutes to read through the following topics that will help to prepare you for your first visit.  If you have any questions or concerns, we can discuss them at the beginning of your first appointment or you can call the office.

If possible, please take the time to schedule your appointments in advance. This will ensure that you receive and retain the time slot that is best for your schedule.  If you need to make changes to an already scheduled appointment, please do so giving as much notice as possible.  Remember, it is your responsibility to cancel any unwanted appointments.   If you are unsure of your appointment, please call the office.

Please understand that we do our best to accommodate our patients.  However, we must balance this courtesy with the busy nature of our clinic and the fact that we hold 1 slot for 1 patient to honor your time – we do not double-book and we allow lots of face-to-face time for practitioner-patient visits.  So that means: a missed appointment is a big loss to our practitioners’ workday. As such, we maintain a firm policy. Please understand we must enforce this policy even when the reason is a personal or occupational schedule conflict.  It is only fair to our practitioners, who have allotted a considerable amount of time in their workday to your care.  We hope that you understand our need to set this firm policy.

*Clients are required to keep credit card or other payment method on file – to be charged in cases of Late Cancellations or Missed Appointments. 

Acupuncture Late Cancel / Missed Appointment Fee: $35 follow-up / $75 New Patient 

Massage Late Cancel Fee: 50% of service charged

Massage Missed Appointment Fee: 100% of service charged

To Avoid a Late Cancellation Fee – 

  1. Contact our office by email, by phone or leave a voice mail, giving MORE THAN 24 hours notice prior to the appointment time.  
  2. Substitute a friend/family member in your place to take the appointment slot.  This is a special allowance that we make to help you avoid a late fee, while honoring our practitioner’s need to make a living.  

ACUPUNCTURE SUBSTITUTIONS:  If the friend/family member is a new patient to our clinic, they will be seen for a New Patient intake interview only (45 minutes).  This will establish him/her as a new patient at our office.  There will be no acupuncture that day, unless time allows.   If the friend/family member is already an established patient, they may receive a follow-up acupuncture treatment that day.  

MASSAGE SUBSTITUTIONS:  The friend/family member would be seen for a comparable service to avoid the Late Cancellation fee. 

We hope this helps to accommodate all the needs of our patients!   Simply let our office know if you will be sending a substitute and make sure he/she comes 15 minutes early to fill out new patient paperwork.

We accept cash, personal checks, Visa, MasterCard, and Discover. We ask that you pay at the time of your visit.  We offer at 15% Prompt Pay Discount for clients who are courteously paying at the time of service.

Health Savings Accounts often allow you to spend your HSA dollars for acupuncture.  Check with your individual plan to find out if acupuncture is eligible.

Insurance companies are starting to provide benefits for acupuncture, specifically when treating certain pain areas.  Please contact your insurance plan by calling the customer service number on the back of your insurance card to find out if you have acupuncture benefits.  If you do, be sure to ask: what pain areas are covered and what your deductible and co-pay or co-insurance might be.  

Our acupuncture providers are in-network with several major insurance carriers in the area.  If you happen to have acupuncture benefits, give our office a call to schedule and be sure to let us know what insurance plan you have.

We consider acupuncture and Chinese herbal medicine to be a very worthwhile investment in your health.  These therapeutic techniques can help you to improve symptoms, feel better faster, and address the root of many problems.  Furthermore, you can prevent major illnesses from happening down the road, by taking care of yourself now.  Millions of individuals in countries all over the world have relied upon acupuncture and herbal medicine as their primary method of health care for centuries. If it has stood the test of time for this long, isn’t it about time that you tried it?

This office uses sterile, disposable needles.

Please make sure you do not eat or drink anything that will stain your tongue for at least an hour before arrival, as we need to examine your tongue. You will be most comfortable if you wear loose, clothing, as acupuncture involves palpation and treatment of various parts of the body. Draping will be provided to ensure modesty. Please refrain from wearing excessive jewelry, as some metals may interfere with the efficacy of the therapy. Also, please refrain from wearing perfume, strongly scented oils and/or lotions on the day of your visit.

-Under the law, health care providers must give patients, who don’t have insurance or who are not using insurance, an estimate of the bill for medical items and services. This is called a Good Faith Estimate.

-You have the right to receive a Good Faith Estimate, and Live Well Medicine, LLC will provide one upon request. The Good Faith Estimate shows the expected cost of a course of acupuncture treatment. This estimate is based on information known at the time the estimate was created.

-You may request a copy of your Good Faith Estimate before or after your first visit, however, we recommend waiting until you have had your first visit with your acupuncturist. This is the most accurate time to receive the estimate because your provider will be able to assess your needs and give you an estimate of how much care he/she thinks that you will need to address your issues. If you wish to receive an estimate in advance of an already scheduled appointment, the estimate will be provided within three days of request and within one business day of a previously scheduled service.

-The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment.

-If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Holistic Nutrition Coaching is offered Virtually through a secure connection hosted by Jane, our scheduling platform. Prior to your appointment, you will receive an email with a link to test your audio and video capabilities. If you are unable to use the video feature, you may choose to conduct sessions by phone.
